By no means is this a complete list. Such deaths are continuing to occur with no clear indication that the military leadership will limit the availability of antipsychotics, antidepressants or prescription "polypharmacy" which can never be scientific, safe or effective. I continue to find 'new' deaths every time I have a spare hour or two to begin Googling again.

I call to the readers attention the posting of Leland Tyrone (#267) which the editor of The Gazette tells us was altered at the military's request. How common or routine are such alterations. How many of these probable SCDs have been called "suicides"? How many accidental or other multiple drug toxicities or overdoses when they are none of these things?


There can be no rational or scientific change of psychiatric drug policy in the US military until such time as there has been a full acknowledgement and explanation of these and all similar deaths in the military-FAB

4. Adams--Sgt. Leonard W. Adams, 42, of Mooresville, N.C., died Jan. 24 2005 in Camp Bucca, Iraq, of a heart attack. Adams was assigned to the Army National Guard's 105th Military Police Battalion, 18th Military Police Brigade, Asheville, N.C. [their diagnosis--……………………………………………………………………………..HA]

5. Adan--Pfc Christopher Adan, 20, was found dead in his room Monday 12 17 07 Others let them flow freely as they paid their last respects to Pfc. Christopher Adan, who was found dead in his room Monday.Soldiers who found him and medical personnel tried unsuccessfully to revive him. [Probable SCD ]

6. Alderman--Pfc Timothy Ryan Alderman, 21. October 20, 2008, the 21-year-old was found dead in his barracks. An autopsy ruled his death a suicide. Found dead dx by EMT with EKG as SCD but called suicide by military [probable SCD]

7. Allison--12 18 2003. Sgt. Glenn Richard Allison, 24, of Pittsfield, Mass., and stationed at Fort Drum, died in Baghdad on Dec. 18 during a physical training exercise, less than a week after arriving in Iraq. The 24-year-old from Pittsfield had a heart attack on Dec. 18 during physical training, his mother said yesterday. Allison, died Thursday in Baghdad, Iraq, after being stricken during a military drill. [ their diagnosis. HA]

8. Anderson, SSgt Cody R, 26: Jan 19, 2010: 26 year old Staff Sergeant Cody R. Anderson was found dead. According to Watertown Police Lieutenant Frank Derrigo, a friend discovered Anderson's body in his apartment at 133 Scio Street. He said there was no evidence of foul play or suicide and that police are awaiting the results of toxicology tests. [probable SCD ]

9. Angelo 2012-01-17: A Marine was found dead in his Camp Pendleton barracks room Sunday, the Corps announced. Pfc. Thomas J. Angelo, 21, of Rochester, N.Y. was an avionics technician assigned to Marine Medium Helicopter Training Squadron 164, Marine Aircraft Group 39, 3rd Marine Aircraft Wing. The cause of his death is under investigation by the Naval Criminal Investigative Service. Prob scd

10. *Apineru, Staff Sgt. Faoa, 32: 7/2/07: was found without a pulse. Marine Maj. Nina D'Amato, who worked with AP for a long time, said the big, bald-headed Marine from Samoa went to sleep around midnight on the 2nd and never woke up. No one knew what caused his death, she said. The results of an autopsy are pending. [ Probable SCD ]

11. Arnn, Spc Jared, 21, Ft. Hood, dod 10 1 18 was found unresponsive Saturday in a barracks room, later died…Another soldier found unresponsive in his barracks, Spc. Jared Arnn, died Saturday at a hospital from internal bleeding from an existing medical
condition, Fort Hood officials said. [probable SCD]

12. Augustin, Adrien E., 20, August 21, 2009, a Marine recruit from Kentucky has died after a training exercise at the Marine Corps Recruit Depot at Parris Island. Marine officials said Friday they are investigating the Monday death of 20-year-old Adrien E. Augustin of Kenton, KY. He collapsed after finishing a physical fitness test and was pronounced dead at a Beaufort hospital. [exertional--Probable SCD]

13. Bachus, Gy Sgt. Christopher Scott, 38, died in his sleep Monday at Camp Lejeune, N.C. In the case of Gunnery Sgt. Christopher Bachus, it is far from clear that he received the least amount of medication possible Bachus received a diagnosis of PTSD, and starting in 2005, doctors put him on a regimen that included Celexa for depression, Klonopin for anxiety and Risperdal, an antipsychotic. In 2006, after a period of stability, a military doctor discontinued his medications. But six months later, Bachus asked to be put on them again.[found dead probable on antidepressant and antipsychotic. According to a detailed autopsy report, his depression and anxiety worsened in late 2006. Yet for unexplained reasons, he was allowed to deploy to Iraq for a second time in early 2007. But when his commanders discovered that he was on psychiatric medications, he was sent home after just a few months, records show.In early March 2008, a military doctor began giving him an opiate painkiller for his back. A few days later, Bachus, 38, called his wife, who was living in Ohio. He sounded delusional, she told investigators later, but not suicidal."You know, babe, I am really tired, and I don't think I'll have any problems falling asleep tonight," he told her. He was found dead in his on-base quarters in North Carolina nearly three days later.According to the autopsy report, Bachus had in his system two antidepressants, the opiates oxymorphone and oxycodone, and Ativan for anxiety. The delirium he experienced in his final days was "most likely due to the interaction of his medications," the report said.Nearly 30 prescription pill bottles were found at the scene, most of them recently prescribed, according to the report. 14. Bailey, USMC Vet Justin, 27: LA VA Rehab Center On 12 March, 2007, was found dead in his room… On Jan. 25, Justin Bailey got prescriptions filled for five medications, including a two-week supply of the potent painkiller methadone, according to his medical records. A day later, he was found dead of an apparent overdose in his room at a VA rehabilitation center on the hospital grounds. He was 27. [probable scd]

15. Baker, Pfc Eli Mundt, 22. January 31, 2008, soldier found dead in the Warrior Transition Unit barracks at Fort Huachuca, Ariz. … Soldier found dead at Fort Huachuca was from Calif., Pfc. 17. Barnhill -Command Sgt. Maj. Edward C., 50, 431st Civil Affairs Battalion, 358th Civil Affairs Brigade, Army Reserve Shreveport, Louisiana. Died of an apparent heart attack after he was found unconscious in a hallway at the Coalition Provincial Authority headquarters building in Baghdad, Iraq, on May 14, 2004. .. Army Sgt. Maj. 18. Beard, Army Sgt. Daniel J., 24. April 08, 2009, Beard had just finished running "a couple of miles" and was in line for a weigh-in when he collapsed. [Probable scd]

19. Bennett, Pfc. Ayonti B., 20, a native of New Orleans, was pronounced dead by German medical personnel, according to Nathan Van Schaik, U.S. Army Garrison Schweinfurt spokesman. He was found dead in his barracks room on January 19, 2011, according to the Schweinfurt garrison public affairs office. The cause of death was unknown, and the incident is under investigation. Bennett was assigned to Troop C, 1st Squadron, 91st Cavalry Regiment, 173rd Airborne Brigade Combat Team. He had returned from a 12-month deployment to Afghanistan in November. [Probable scd]

21. Bezanson, Spc Caleb Allen, 28 (May 21, 2009), was found dead by fellow soldiers in his barracks. [probable scd]

22. Binnier, Capt. Felicity Binnier. 40 FORT McCOY, Wis. - 8 4 11. found dead at Fort McCoy in Wisconsin over the weekend. The fort said Capt. Felicity Binnier of East Stroudsburg, Monroe County, was found unresponsive at the Army base Saturday morning. Binnier, 40, was at Fort McCoy in west-central Wisconsin for annual training. 23. Bishop, Spc. Steven Jesse, 29, of Christiansburg was assigned to the Army Reserve as a civil affairs soldier. The defense department announced Wednesday that the 29-year-old soldier died on March 13, 2010, at Contingency Operating Base Speicher in northern Iraq of a non-combat-related illness. Bishop is a 1999 graduate of Floyd County High School and joined the Army Reserve in April 2008. "due to cardiac arrest" [probable scd]

25. Blevins, SPC Joe Trinity, 29, Jun. 18 2010, found unresponsive in his barracks' room and transported to Carl R. Darnall Army Medical Center, Ft. Hood, where he was pronounced dead. [ probable scd]

26. Bloomquist, Pvt Michael, 19, Jan 24, 2009. Soldier found dead in barracks room… HINESVILLE, Ga. - The body of a Fort Stewart soldier was discovered in his barracks room Saturday night, an installation spokesman confirmed Wednesday.Army Pvt. Michael A. Bloomquist, 19, was found in his room Saturday evening. Installation spokesman Kevin Larson said military police are still investigating the cause of death. [prob SCD]

27. Boles, Sgt. Dennis J., 46, of Homosassa, Florida, died October 24, 2004 in Camp Arifjan, Kuwait, when he was participating in a 10-mile road march and collapsed. Boles was assigned to the Army National Guard's 171st Aviation Battalion, Brooksville, Florida. [Their witnessed description-Probable scd]

30. BOWER 07/23/11WEST POINT - A Department of the Army investigation will determine how freshman Cadet Jacob D. Bower died Thursday.On Friday, unnamed military officials told NBC News they believed Bower was a victim of this week's intensely hot weather, but an autopsy will be needed to say for sure if that's true.The 18-year-old from Fairmont, W.Va., was found unresponsive while taking part in a training exercise around 6:05 p.m. Thursday. 31. BRADEN Capt Michael, FORT RILEY, Kan. - DOD 4 20 12 A U.S. soldier based out of Fort Riley, Kansas, has died in Afghanistan, the Army announced on Friday.Capt. Michael Braden, 31, was pronounced dead at a medical facility on Bagram Airfield, Afghanistan, after being found unresponsive in his living quarters Wednesday morning. 33. Brandt, Petty Officer 3rd Class Matthew David, 22, of Nashville, Tennessee, January 19, 2005. A sailor was found dead in his barracks at Guam Naval Base. The cause of death is still under investigation by the hospital medical examiner. [would be interesting to know how signed out in fact all of their final official DOD VA diagnoses must be compared [PROBABLE SCD]

34. Britt, Pvt. Jamal, 19, Oct 07, 2009. A Fort Jackson soldier, who collapsed while taking a physical fitness test, died of heatstroke, the Richland County Coroner ruled Friday. The victim, Pvt. Jamal Britt, 19, of Edgewood, Md., collapsed Wednesday afternoon, the Army said. He is the third soldier to die in basic training at Fort Jackson since Aug. 20, and the sixth in the past year. 36. Bronaugh, Spc. Blake, 25, Nov 30, 2008, was found dead in his barracks from unknown causes. Spc. Blake A. Bronaugh of C Company, 864th Engineer Combat Battalion (Heavy), was found dead in his barracks room at Fort Richardson in the late afternoon Nov. 27. [probable scd]

38. BROWN CHRISTOPHER JAMES JOHNSON CITY, NY -- A paratrooper stationed at Ft. Bragg was found dead last week in New York.Officials with Fort Bragg say 22-year-old Sgt. Christopher James Brown was found dead during the early morning hours of Dec. 30, 2012 in Johnson City, NY. Brown is a native of Johnson City. His cause of death is currently under investigation by the Johnson City Police Department. Officials say Brown recently separated from the military under honorable conditions and was in Johnson City on terminal leave. PROB SCD

39. Burdette, Pfc. Brandon L., 2 22 11 22, of Ashland, Ohio, was found in his barracks room in Schweinfurt, Germany and died at the scene. He was assigned to Bravo Battery, 1st Battalion, 77th Field Artillery Regiment. The cause of death is still under investigation. [FOUND UNRESPONSIVE IN BARRACKS--PROB SCD ] ]

40. Bush, Pvt Matthew D., 20. A fellow soldier tried to wake Bush and noticed he was not breathing, Aug. 8 2003 in Kirkush, Iraq, of what is believed to be a heat-related illness. [heat-related it is said, probable scd]

41. Buskey, PFC Robert, 27, 3/2/09 died in his sleep on March 2, two months before he was supposed to leave for Afghanistan. [probable scd]

42. Cameron 2 8 08 A Fort Wainwright Soldier was found dead in his barracks room Saturday. A fellow Soldier found Spc. Aaron Cameron unresponsive and called for emergency medical services. Cameron, 22, of Marietta, Ga., was an infantryman who joined the Army in September 2005 [ probable scd]

45. Carlson, Spc. Frederick A., 25, died of a non-combat-related cause in Taqqadum, Iraq, on March 25, 2006. "Huffing" caused guardsman's death. A Pennsylvania National Guard soldier, who died in Iraq this spring, accidentally killed himself while inhaling from a container of pressurized air to get high, an Army investigation concluded. He was found unconscious in his room at the base in Taqqadum shortly before 6 p.m. on March 26, 2006. Carlson went to sleep after returning from a mission at 6 a.m. and woke up around 4:30 p.m., investigators said. He was found unconscious a little more than an hour later and could not be revived. 46. Carney Spc. Crystal Joy, 21, FORT HOOD (May 21, 2009) of Houston was discovered unresponsive in a house on Sunday by another soldier. "+The incident remains under investigation. Spc. Carney makes the fifth soldier discovered dead on post this year (2009). Earlier in the week, Army officials released the name of Spc. Caleb Allen Bezanson, (see above) 28, of Ridgecrest, Calif., who was found dead by fellow soldiers in his barracks Monday. Two other soldiers were found dead in their barracks in January and a third was found in March. Authorities were investigating at least five other soldiers' deaths last year"-[probable scd]

47. Carney Sgt. Kendale B., 28, from Hamlet, NC, was found dead at her mother's home on August 16, 2008. Carney was on leave from her unit in Iraq, where she was on her second deployment. Her death was ascribed to natural causes. [Probable scd]

48. Carter, PFC Jonathan C., 24 January 22, 2009. Carter, who is based at Fort Bliss in El Paso, collapsed during training around 6:30 a.m. Sunday. [Probable scd]

49. Cassidy, Sgt. Gerald, who was in one of the transition units, was found dead in his room Sept. 21 2007 at Fort Knox, KY. , who was in one of the transition units, was found dead in his room Sept. 21 2007 at Fort Knox, KY, about 15 months after being wounded by a roadside bomb in Iraq. An autopsy later determined he had been dead for hours and might have been unconscious for days before he was found alone. The Army Criminal Investigation Command determined the death was accidental and caused by a multidrug toxicity complicated by severe atherosclerotic coronary arterial disease. Paul Boyce, an Army spokesman, said Friday that Surgeon General (Army) Schoomaker's intent by the comments was to educate about the growing health risk of overdoses in the military population and the American population as a whole. Sgt. Gerald Cassidy died after neglect at Army hospital A sergeant in the Indiana National Guard, G.J. had been injured in Iraq by a roadside bomb in June 2006. He returned to the U.S. five months ago, and was receiving inpatient medical care through the "Wounded Warrior Transition Programme" at Fort Knox, Kentucky. He told his mom he hadn't slept in days. Then two days passed, and neither Kay McMullen nor G.J.'s wife Melissa had heard a word from him. The silence was eerie. G.J was bored at Fort Knox and called them three or four times a day. He almost always answered his phone and told them many times he couldn't wait for his medical hold to end, so that he could get back to Indiana, where he planned to help raise his children and re-enter the civilian workforce as a high school teacher. Normally, he would call his family 3-4 times a day to talk to them. But when he did not call, the family started calling the hospital at 8 am on September 21st. [Probable scd]

51. CHAMBERS JUSTIN 3 10 12 DOD A soldier assigned to the Raymond W. Bliss Army Health Center on Fort Huachuca was found dead in a Sierra Vista apartment early Saturday morning. from Muskogee, Okla. [PROB SCD]

54. Claunch, Master Sgt. Herbert R., 58 , of Wetumpka, Ala., died April 18, 2004, in Guantanamo Bay, Cuba, after collapsing on the floor in his quarters. [probable SCD]

56. CLAWSON, JEREMY ANDREW, July 23, 2009 FORT SILL -- Army officials identified 1st Lt. Jeremy Andrew Clawson, 36, as the soldier found dead Tuesday at Fort Sill. A 911 caller reported finding Clawson's body Tuesday afternoon along Mow-Way Road, the military said. His death is still being investigated and authorities have not said if they suspect foul play. Clawson was a Kansas Army National Guard member and served tours of duty in Afghanistan and Iraq, the military said. Funeral services are pending [prob scd]

59. Conant Spc. John, 36, DOD 4/9/08 Fort Carson. The death at his off-post home was determined to have been from natural causes, in April 2008, after taking morphine and Ambien, the sleeping pill, he died in his sleep. A coroner ruled that his death was from "natural causes." [ probable scd]

60. Conley. Two Fort Riley soldiers found dead in separate incidents Tuesday, July 12, 2011 Spc. Nathan Conley, 21, was found dead in his barracks room at the Warrior Transition Battalion on June 27, according to a press release issued by the 1st Infantry Division (other is Aaron Evilsizer though dead of gunshot) . His death was investigated and later determined to be suicide. Conley joined the Army soon after graduating high school in 2008. The Battle Creek, Mich., native served in Korea as a M1 Abrams tank driver before being stationed at Fort Riley with the 1st Infantry Division. He was deployed to Iraq from November 2010 to February 2011 in support of Operation New Dawn. The Warrior Transition Battalion's mission is to support the rehabilitation and successful transition of wounded, ill and injured soldiers back to active duty or to veteran status. 61. Consolacion, Spc Pedro A., February 8, 2004, Fort Rucker, Ala., was found dead in his barracks room at an Alabama Army base. [Probable scd]

62. Coons, Master Sgt. James C., 35, 385th Signal Company, 54th Signal Battalion Conroe, Texas. Diagnosed with post-traumatic stress disorder after serving in Iraq, Coons' body was found at an outpatient hotel at Walter Reed Army Medical Center on July 4, 2003. In December 2005, a military casualty board ruled that his suicide should be considered a casualty of war. [probable scd]

63. COOPER Spc. Christopher M. Cooper. The Defense Department has identified a soldier found dead last week in his quarters in Iraq. Spc. Christopher M. Cooper, of Oceanside, Calif., died Friday of injuries sustained from a non-combat related incident. Cooper, 28, was assigned to the 2nd Battalion, 28th Infantry, 172nd Infantry Brigade, Grafenwφhr, Germany.The incident is under investigation, officials said. [Prob scd]

He returned from his second deployment to Iraq complaining of back pain, insomnia, anxiety and nightmares. Doctors diagnosed post-traumatic stress disorder and prescribed powerful cocktails of psychiatric drugs and narcotics. Yet his pain only deepened, as did his depression. "I have almost given up hope," he told a doctor in 2008, medical records show. "I should have died in Iraq." Airman Mena died instead in his Albuquerque apartment, on July 21, 2009, five months after leaving the Air Force on a medical discharge. A toxicologist found eight prescription medications in his blood, including three antidepressants, a sedative, a sleeping pill and two potent painkillers. When Openshaw returned home from treatment in Florida in April 2008, his family and friends were buoyed by hope that he had turned a corner. He was still heavily medicated, however, with narcotics for the lingering pain from his parachute accident and antidepressants for his post-traumatic stress disorder. In April, Ort reported her medicines had been stolen from her at Fort Irwin, a training base in Southern California. Her prescriptions were not refilled. Instead she was given four new medicines, including morphine. On May 3, three days after Ort was prescribed the potent new batch of painkillers, her bunk mates heard her moaning in her sleep at dawn. At 7:10 a.m. she was found unconscious on her cot. Ort, a 24-year-old soldier with a 16-month-old son, was pronounced dead an hour later. A coroner's report concluded an "accidental overdose of prescription medications" caused her death. The night before she died, she had talked to her mother, according to the San Bernardino County sheriff coroner's investigation. Vickey Ort told the coroner's investigator that her daughter did not sound like herself, that she was "drugged up" and slurring her words, and that her daughter admitted she was "out of it" because of her new medicines. The coroner's report noted that three days before her death Ort had been prescribed three painkillers - morphine sulfate, gabapentin and hydrocodone - and a muscle relaxant marketed as Flexeril. A Marine corporal from Brevard County died as he tried to swim Iraq's Euphrates River on a covert mission, officials said.The body of Dustin H. The study, based on a survey and medical data from the First Marine Division, also cites the ephedra-related death from a cerebral hemorrhage of an unidentified service member at the Point Loma Naval Submarine Base. The military study is the latest to take aim at dietary supplements that contain the amphetamine-like herb. The products, made by companies such as San Diego's Metabolife International, have been linked to hundreds of deaths, serious injuries and side effects. Earlier this week, a General Accounting Office review of thousands of consumer health complaints about Metabolife's best-selling Metabolife 356 cited 92 serious complaints, including five deaths, 18 heart attacks, 26 strokes and 43 seizures.
The Camp Pendleton study, conducted by Capt. Eric C. McDonald, a surgeon in the First Marine Division, has not been published in a peer-reviewed journal. One body was found at 10:30 Tuesday night. The second body was found at 8:20 Wednesday morning. According to the Northwestern District Attorney's office, the cause of death will not be known until autopsy and a toxicology report is done, but they did confirm that these were non-violent deaths and there was no foul play involved. We do not know the ages of the men, but it is likely that they are veterans because that is who this shelter serves. The VA Medical Center is a federally owned and operated facility for veterans. But, Soldier On is a non profit organization that leases this property from the federal government. 88. The bodies of two fishermen, who were also military veterans, were found in a pond on Sunday. Authorities identified the victims as Richard Maple, 64, and Isiah "Sonny" Macon, 59. Both men were from Charlottte.The men were fishing Saturday on a neighborhood pond behind Macon's home on Williams Road off East WT Harris Blvd.Search crews found Maple in the pond around 10:30 p.m. Saturday and Macon around 1 a.m. Sunday.Macon joined the Navy after graduating from Harding High School.
